Medical Affairs Root Cause Analysis Meeting
The meeting was held to perform a root cause analysis of an adverse event involving the management of an ectopic pregnancy. The discussion centered on the misinterpretation of ultrasound findings, which led to the inappropriate recommendation of methotrexate therapy instead of surgery, despite the patient's condition presenting relative contraindications. Attendees reviewed the clinical timeline, evaluated the decision-making process, and identified communication failures and adherence to protocols as key areas for improvement to prevent similar occurrences in the future.

The board meeting primarily focused on a proposal to implement a preterm birth prevention program involving the administration of 17 hydroxyprogesterone injections for high-risk patients. A clinical presentation detailed the efficacy of the medication in reducing premature births and associated health complications. The chief financial officer provided an analysis of the high costs associated with the drug and administrative expenses, which raised concerns regarding the clinic's budget and operational sustainability. The board discussed the potential trade-offs, noting that funding this program would require delaying other initiatives, such as the centering group prenatal program and a transitional home health program. It was concluded that a formal cost-effectiveness analysis is required to determine the best path forward.
